Conditions of Attendance: I do hereby release, discharge, indemnify and hold harmless Center Island Sports Camps, and its owners, directors, officers, employees, agents, successors, and assigns from and against, and waive any and all claims or liabilities for, any injuries, losses or damages, including without limitations, injuries to my child, my self and/or property, arising out of or incident to my child’s participation in Center Island Sports Campss Summer Camps. I hereby state that my child is medically cleared for sports activities and has no underlying medical conditions which would affect his/her participation. All payments are non-refundable including cancellation due to inclement weather. Camper may apply registration fee towards future events. Permission is hereby given to Center Island Sports Camps to use photographs and/or endorsement of the campers. Center Island Sports Camps is not responsible for any lost or stolen property.
